Our Mentoring Services focuses on providing a safe and stable transition to the community for our participants with complex support needs. As our Program Manager, you will focus on encouraging support that will foster independence and enhance opportunities for our participants. With strong leadership and guidance, you will ensure that your team provides person centric support that will assist our participants to achieve their interests, needs and aspirations.Please note that this is a 3 month full-time fixed term contract. Key Responsibilities:Ensure staff work in consultation with each person, families and stakeholders to develop and review Person Centred Plans/ Transition Plans.Monitor staff performance to ensure ‘positive approaches’ for the management of challenging behaviours.Support staff to establish and maintain effective lines of communication with parents, families, carers, and advocates etc.Work towards breaking down negative community values.Participate in staff training and development as required.About You:You are a collaborative leader who is passionate about supporting our operational teams. Having worked within the Disability/Nursing/Community or a similar industry, you are fully equipped with in-depth knowledge of the issues, trends and philosophies underpinning the provision of services to persons with an intellectual disability and/or psychosocial disability.
